We are here to help serve state of Utah employees. Employees (those with a utah.gov or other state issued email) are encouraged to submit questions or tickets through AskHR. Questions are typically answered within one business day or less, during the business hours of 8:00 a.m. to 5:00 p.m., Monday through Friday (except observed holidays).

For any verification of employment for State of Utah employees, please contact the Employee Resource Information Center (ERIC) at 801.957.9390 or you can fax us at 385.465.6054, or email to [email protected].

In order for the ERIC to complete a written verification of employment for an employee, we must have at least the last four digits of the employee’s social security number, and the request must be an authorization form that is signed by the employee. Information we can provide in a written verification of employment:

Dates of Employment
Salary (including hourly rate, current and prior yearly salary)
Average hours worked
Date of last pay increase
Amount of last pay increase

FOR W-2 FORMS
Duplicate W-2's can be requested after March 1st directly from the Division of State Finance. To request a W-2 send an email to [email protected], you MUST include the following in your email:

Your full name
The W2 year you are needing
The last 4 digits of your social security number
Your date of birth
Your current full address

Current state employees should contact their supervisor, their assigned HR field team, or the ERIC (Employee Resource Information Center) with any questions or assistance requests. To contact the ERIC, use the AskHR link above.
