Use this form to add new or updated direct deposit accounts for your paycheck. Additions and updates can also be made electronically in the Employee Self Serve system. Completed forms must be submitted to State Payroll via fax to 801-538-3244.

Finance forms are fillable PDFs. Forms ending with an asterisk save information to and retrieve information from their database and require a login to access. These forms must be opened in a browser other than Chrome.
