The Division of Human Resource Management’s (DHRM) service level agreement with all customers requesting a verification of employment via fax or email is to produce results by the end of the following business day. If you have not received your verification results, or have not received another form of response within this time frame, please call the ERIC at (801) 957-9390.
Verbal Verification of Employment Guidelines:
When you call in a request for verification of employment we can only verify information that the employee has already submitted to you. We will not provide any new information unless you send in a written request via email or fax. Often times, if the written request is sent by email, the additional information (such as dates of employment, position or salary) can be provided while you are still on the phone. If you are inquiring about work location, or phone number that has not been provided to you by the employee, you can refer to utahsright.com for further information.
To expedite the process, please have the following information readily available concerning the employee whom you would like to verify employment:
- First and Last Name
- Full Social Security Number
- Date of Birth
Written Verification of Employment Guidelines:
In order for the ERIC to complete a written verification of employment for an employee, they must have at least the last four digits of the employee’s social security number, and the request must be an authorization form that is signed by the employee.
Information we can provide in a written verification of employment:
- Dates of Employment
- Salary (including hourly rate, current and prior yearly salary)
- Average hours worked
- Last pay increase
- Amount of last pay increase
