To help you find out more about your benefits you may access the 2015-16 Benefits Summary Book to learn more.  If at any point you have questions you may contact a human resource representative at the Employee Resources Information Center (ERIC) by calling (801) 538-3742 or by submitting your questions online using AskHR.

 

**Important Deadlines**

  • Traditional Medical Plan Enrollment – within 30 days of hire or eligibility date
  • High Deductible Medical Plan Enrollment – within 60 days of hire or eligibility date
  • Dental Plan Enrollment – within 60 days of hire or eligibility date
  • Voluntary Benefit Enrollment60 days of hire or eligibility date
  • Short Term Disability Insurance (Optional)60 days of hire or eligibility date for coverage without late enrollment penalties
  • Retirement Enrollment – Automatically enrolled in Hybrid Plan but have one year to finalize decision  (Tier II Employees only)
  • Statewide Required Training – Must complete all required training within two months of hire date

  • Benefits in Brief Training Session – Monthly NEO orientation class provided to all newly eligible employees.  Representatives from Public Employees Health Plan (PEHP) and Utah Retirement Systems (URS) provide information about each plan available and help you with questions related to your benefits.  You will need supervisory approval to attend and you may register for the next available class here

  • Employee Resource Information Center (ERIC) – a human resource technician is available Monday through Friday, 8:00 a.m. to 5:00 p.m. to assist you with your benefit questions at (801) 538-3742, or you may submit questions electronically using AskHR 
  • Public Employees Heath Plan (PEHP)  – you may contact PEHP directly by calling 801-366-7555 or by visiting their website at www.pehp.org
  • Utah Retirement Systems (URS) – you may contact URS directly by calling 801-366-7770 or by visting their website at www.urs.org
  • 2015-16 PEHP Benefits Summary Book – Effective 7/1/2015 – PEHP has provided a benefits summary book for all new employees to review coverage options and premiums for the Fiscal Year 2016

Non-benefited Employees Please Note:  Even if you are not a benefits eligible employee, you are still eligible to enroll in a 401(k) retirement savings plan. To participate, please use the URS Enrollment Contract for 401(k) or 457 Plan to learn more about investment options available to you and to enroll. You may also enroll online at URS.org.
