URS Statement of Ineligibility

Utah Retirement Systems (URS) requires we notify a new employee they are not eligible for a retirement contribution made on their behalf by the state but that they can still opt to contribute to a 401k plan on their own.  Employee must select whether they would like to contribute or not and sign this form to be submitted to URS.
